 Healthy Clones?

Anyone ever had problems where their clones look sorta unhealthy and sometimes die before rooting and even after rooting sometimes in dirt? I've seen plants that were looking a little rough bounce back though but I'm wondering does this tend to stunt them in the long run if they are struggling as clones?
I've seen some clones come out perfect, and stay completely healthy, but I don't remember anything special being done. Is it just luck, or is there really a certain way to make sure clones stay a healthy green the whole time?

If there are no pests the most common reason they don't root is bacteria or when the roots are starting thing go dry or are to wet and they rot. There is a product called hydroguard that will take care of the root rot part and the bacteria. Once you get the hang of it you can pretty much take any size cuttings you want.

How do you determine how much water is TOO much water? I've been told to keep them wet while they are in the rooting process, but not to give them any nutrients during that time. Just water.

some clones that i took about a month ago are starting to look really deformed. i took them at around 2 -3 weeks after flowering was induced.

most likely due to the time period you took your clones in. You've basically forced plants which was using hormones to flower, to a.) revert back to veg state, b.) produce roots, then c.) produce the needed hormones to maintain itself in a veg state.
EG you've caused your clones to go through hella stress.

Not in my experience. I also see no reason why they would unless they stay smaller than other plants and receive less light.
