 Air duct help. how do I attach it to a wall?

Alright, whats the piece called thats square and attaches to a round air duct to allow it to go through the wall?
Id prefer if it had slanted vents.
I need it to recieve a 6" duct and on the otherside be an outlet.

I can't recall the 'name' of the piece, but you can buy them at Home Depot or Wal*Mart inexpensively.

I think it's called a flange, but I'm not positive.
