Is CalMag nessesary for every watering with nutrients or just when the plants have cal/mag problems, in soil?

It depends what youre growing in and what kind of water and nutes youre using.

tap water usually has plenty in it. depends on your soil, but probably not unless you have issues. I added it out of curiosity but didn't notice any difference so I stopped using it. my tap water is at 150ppm and is fine without it in hydro (coco, drip feed). I still have the bottle so every once and awhile in early development I'll give it a dose, but as soon as the bottle is gone I probably go without.
