So he trembles because of how your body reacts to that "sensation" and he jerks his head voluntarily to shake that feeling. Or is it involuntary jerks?

this happens to me if i yawn and stretch really hard.
if i'm standing i'll collapse on the ground and have small convulsions.
it's pretty fun and it's happened since i was a child.
natural high lol.

Once I ripped a bong alot bigger than my usual and as I exhaled I could feel myself kinda running out of breath but it felt like I still had alot more smoke in my lung, and as I finished exhaling my head kinda jerked from side to side, not like a seizure, but its kinda looked like I was strongly disagreeing with something.
It was weird but, but I kept smoking and it hasn't happened more than like 2times since, and they were both much larger bongs than I'm used to

Clearly no one read what I had to say. IT is my professional opinion as a former EMT that your brother should see a doctor. This could be something much larger and very important. I understand the insurance and all that. But that being said if this were to do irreverisable damage then the lack of insurance will still be an issue with far greater medical bills. Or don't listen to me. But at the very least stop smoking weed. Its not good to black out like that, its not fun, and just plain stupid to continue doing it.

oxygen deprivation maybe 
does he still black out when smoking a joint?
people seem to take smaller hits from joints and always inhale a fair amount of air with the hit so who knows.
